This packet can be completed by hand or typewriter, use fill in the blanks, and You can use this packet to ask for
a court order to change your name if age 18 or over. There is separate packet for changing the name of your
child under the age of 18. Please specify adult or minor. Normal delivery is approximately 10 to 14 days. The
cost per packet is $39.99 + $3.00 S/H. This is for NEW JERSEY ONLY. Others to be added later.
