information and belief, and we hereafter submit this Notarized Affidavit, notarized by a person authorized by our
state, to take oaths and verify our identify, in support of our Application for issuance of a Muslim Marriage
Certificate from Muslim Vital Records International issued on fine parchment with raised seal and signature of
